Are you wondering what exactly the Injured effect means in Diablo 4 for both the player character and enemies? Don't worry, Nephalem; we will make this status effect abundantly clear for you.
This guide explains the Injured status effect in detail, which you might see appear on items and skills in Diablo 4, complete with the percentages and how it translates into gameplay.
How Does Injured Status Effect Work?
When you start gearing up in Diablo 4, you might find items with affixes that have the "While Injured" effect. For example, Polearms all have an affix that grants an increased percentage of damage to Injured enemies. Another example is the Barbarian's Slaying Strike in the skill tree, which adds 7.5% damage to Injured enemies.
You will also find the Injured status effect on some Glyphs and Legendary Aspects. It is an important effect that Diablo 4 players need to be aware of, so check out a very quick explanation below:
Injured means the enemy or your character is below 35% health. At this point, effects from affixes that have "When Injured" are applied.
So what does this mean in terms of gameplay? Well, the effects get applied automatically to enemies. For your player character, we suggest avoiding big damage hits by using Health Vials before your character goes below 35% health. Going into big fights against strong elite packs or bosses can therefore be prepared for by using skills or triggering Fortify/Barriers, giving yourself effectively a larger health pool.
Alongside the Injured status effect, you will sometimes also find a Healthy status effect. These effects apply when you are at 80% Health or above, and the same goes for enemies!

Nothing should change about this status effect, but there's always a chance Blizzard might adjust the percentages. If anything does change, we will endeavor to update this article accordingly.
